Step 3-II: Occlusion of IVC and left renal vein.

• Tourniquets should be fastened in the following order: IVC below renal veins, left renal vein, IVC above renal veins; complete occlusion of IVC is obtained

Step 3-III: Cavotomy.

• Incision of IVC with cold scissors and careful removal of the entire tumor thrombus
• Incision of IVC should be close to right renal vein in order to maintain sufficient IVC lumen after the excisionv
