After releasing the external iliac vein, the lymphatic block is dissected along the lateral pelvic wall in the fossa obturatoria. Small vessels are cauterized. Slight medial

tension on the already mobilized tissue allows exposure of the underlying obturator nerve. The lymph node tissue packet located in the obturator fossa is dissected, completely skeletonizing the obturator nerve and vessels. Particular care should be taken to avoid mechanical and thermal injury to the nerve.
